Website Structure
This highly depends on your current website structure, but ideally you should have the most important info easily accessible.
After setting up the Welcome Packet, Intake, RSVP, and **Feedback Forms** (5 minutes each), you’ll have four important links to put at the top**.**
Website Menu
Ministries>”THRIVE” Disability Ministry>Landing Page
Landing Page Contents
- “THRIVE” Welcome Packet
- Accommodations Request Form
- Buddy RSVP Form
- Anonymous Feedback Form
- Disability FAQ
- Buddy RSVP FAQ
Page Organization
- Depending on your website setup, this FAQ can get long.
- Due to this, you could put the Buddy FAQ in the welcome packet or on it’s own page with the Buddy RSVP.

If using Planning Center (or related software), all forms must be built inside your account.
-
Click below for a simplified software template with directions on field types etc.
Disability FAQ (Software Template).pdf

- Replace all bolded/CAPS words with your church info.
- As you fill in your church specific info, delete any questions that are not applicable.
