• Procedures, point of contact, and pertinent info need to be clear and easily accessible. </aside>

Prerequisites

After setting up the Intake, RSVP, and **Feedback Forms** (5 minutes each), you’ll have three important links/QR codes to include**.**

• Accommodations Request Form (Friendly naming Disability Intake Form)
• Buddy RSVP Form
• Feedback Form

*****If you want to print out the Welcome Packets, you’ll need to make QR codes for each link.

******Otherwise, the PDF download on your website will have clickable hyper-links.

1. You’re trained for and supportive of people with disabilities
2. You’re organized and responsible to take care of them/their family
